Have you or your team used data to make significant positive changes to the identification and/or management of people living with diabetes through service redesigns, clinical prioritisation of patients or a shift or change in resources?

The backlog of care in resource stretched healthcare systems requires innovative approaches to aid clinical prioritisation. Informatics tools, machine led learning or Artificial Intelligence (AI) may assist to identify and prioritise people with diabetes who are likely to deteriorate whilst awaiting an appointment to optimise clinical outcomes and resources.

This category recognises initiatives that have used data /health informatics that can enhance diabetes care and improve operational efficiency. Examples of data and information that is freely available to the NHS such as electronic health records, NDA, QOF, HES, DOVE or data and/or analysis can be provided by 3rd parties. You may have used 1 or multiple data sources or combined your data to support prioritisation of patients. The key with this category is the data led decision that took place must have led to a change in service, resource, investment that is of the benefit to patient care.

  1. Describe the identified need, challenge and context for the initiative
  2. Demonstrate evidence of health informatics approach: what did you do differently, describe how you adapted and changed with clear objectives and measurement tools to include safety considerations, clean and validated the data, develop assumptions and risk categorisation if necessary.

  1. Project effectiveness to illustrate demonstrable quality and efficiency improvement(s)
  2. How the initiative brought about change to support improved outcomes and what was the impact for people with diabetes?
  3. Cost efficiency and economic viability: indicate how your project was funded, evidence of, and maximising, quality health outcomes and minimising NHS costs, getting
    better value from the NHS budget.
